Palliative Care Clinic
Partners In Home Care is preparing to expand our Palliative Care services with the launch of a dedicated outpatient clinic in Missoula. Located at our Palmer Street office, this new clinic will offer compassionate, team-based care for individuals living with serious or life-limiting illnesses. It is designed to help patients manage symptoms, navigate treatment decisions, and improve quality of life throughout their health journey.

Current Palliative Care patients can now schedule visits at the clinic, allowing them to access in-person support and guidance in a comfortable, dedicated space.

Our experienced team of nurse practitioners, physicians, social workers, and chaplains will provide personalized care that reflects each patient's goals, values, and needs.

Goals of Care Discussions
We help patients and families think through the kinds of decisions that may arise over the course of an illness and encourage proactive conversations about care preferences. These preferences are often documented in an advance directive, this is a legal document that communicates your values and wishes, which can be updated as your health or outlook changes.
Guided Transitions
If and when hospice becomes an appropriate option, our team helps patients and families transition smoothly. We offer guidance, answer questions, and coordinate care to ensure a compassionate, well-supported shift in focus.
