Home and Community Based Services (HCBS) provide opportunities for Medicaid beneficiaries to receive services in their own home or community rather than institutions or other isolated settings.

Home & Community-Based Services is a dynamic program that enables older adults and persons with disabilities to live fully integrated lives within their own communities. Social Work Case Managers and Registered Nurses work closely with members using a strengths-based model to design individualized service plans.

Eligibility Requirements
-
Clients must be Medicaid eligible;
-
65 years or older, or under 65 with a physical disability; and
-
Meet Nursing Home level of care.
