Our outpatient Palliative Care Clinic is designed to support you and your loved ones in managing the symptoms and challenges of serious illness. Whether you're seeking help with pain control, navigating treatment options, or discussing future care wishes, our team is here to provide personalized care in a peaceful, welcoming environment.

  • Advance care planning, including POLST and directives

  • Emotional and spiritual support

  • Coordination with your existing doctors and specialists

  • Help understanding treatment options and goals of care

Our Palliative Care Clinic is open by appointment only. Drop-in visits are not available.

  • Current Palliative Care patients can call us to schedule an in-person clinic visit.

  • New patients require a physician referral to be seen in the clinic.

​How to Find Us:


Suite 103 is located inside our Palmer Street office. The exterior entrance to the clinic can be a bit tricky to spot, we recommend entering through the main building door. Once inside, follow the signage to the Palliative Care Clinic, via the CPAP Clinic/Home Medical Equipment entrance (Suite 101). Our staff will be happy to guide you.
