Palliative Care
Palliative Care is a specialized area of medical care that focuses on providing comfort, support, and symptom control to patients with serious or life-limiting illnesses.

Palliative Care aims to improve the quality of life for patients and their families by managing symptoms, providing emotional and spiritual support, coordinating care with other healthcare providers involved in treatment, and respecting patient preferences related to goals of care. Palliative Care can be provided at any stage of a patient’s illness.

Goals of Care
Goals of Care involve learning about the types of decisions that might need to be made related to illness trajectory, considering those decisions ahead of time, and communicating your preferences. These preferences are often put into an advance directive, a legal document that allows you to express your values and desires related to care, one that you can adjust as your situation changes because of new information or a change in your health.

Continuum of Care
The goal of Palliative Care is to provide care that can continue without interruption and is tailored to a patient’s evolving needs.  Palliative Care can meet the patient’s needs at any stage of illness and can facilitate discussion regarding when a shift to end-of-life care may be a consideration.

Symptom Management
Palliative Care symptom management is a holistic approach to treating symptoms and improving quality of life for people with serious illnesses. Â Palliative care plans may include steps to address symptoms through medication management, care techniques, or health care coordination.

Family Support
Palliative Care can provide family support in a number of ways, including helping to understand the patient’s wishes, relieving the burden of patient care, facilitating resources to help with tasks, and referring to professional support when indicated.

Mental Emotional, and Spiritual Wellbeing
Palliative Care can provide resources to help cope with the psychological aspects of living with serious illness. Â This may include formal psychotherapy and spiritual care, or more informal support including listening, creating a supportive environment, and connecting a patient with resources.

Treatment Planning
Palliative Care treatment planning involves creating a plan to improve a patient’s daily life by preventing or easing suffering.  The plan of care guides palliative treatment and takes holistic care of the individual into consideration.
