Partners In Home Care, Inc. is a non-profit organization co-founded by Community Medical Center and St. Patrick Hospital and Health Sciences Center. We provide a variety of home care services to people all across western Montana. Services include:
Home Health Care
Our home health care program offers a broad range of services for people who need health care services in the home. Our team of nurses, social workers, nurse aides, and rehab professionals (including physical, speech, and occupational therapists) help with the transition from the hospital or nursing home back to the primary residence. We enable our clients to recover and/or manage health problems in the comfort of home. Our staff works closely with each client and physician to develop a plan to meet a wide variety of health care needs.
Hospice & Palliative Care Services
Our hospice team offers physical, emotional, and spiritual support to terminally ill persons and their families. Hospice and palliative care is available 7 days a week, 24 hours a day. We take a wholistic approach that emphasizes pain control and symptom management as well as emotional and spiritual health. Our ultimate goal is to enhance quality of life for the patients we serve. We also offer a wide range of bereavement services for families and friends.
Home Infusion Therapy
Our infusion therapy program includes intravenous medications, state-of-the-art equipment, professional IV nurses, and clinical pharmacists. We can provide chemotherapy, antibiotic therapy, and much more. We train clients to help themselves with pain and nausea management. Also, our trained reimbursement specialists help clients with insurance verification and other reimbursement issues.
Case Management
Our case management teams facilitate the Montana Medicaid Waiver Program's Home and Community Based Services. This program is available for Medicaid-eligible persons who are 65 years or older and who need long-term care in order to remain in their homes. These services are also available to non-Medicaid persons on a private pay basis.
Home Medical Equipment
Our home medical equipment team provides a full line of medical equipment for the home, including respiratory services. We are committed to helping our patients achieve independence and the highest quality of life. We strive to provide excellent customer service, which includes actively communicating with your health care team to make sure you receive the best health care possible.
